הגשת בקשה רשמית למימוש זכויותיכם בהתאם לתיקון 13 לחוק הגנת הפרטיות
בהתאם לתיקון 13 לחוק הגנת הפרטיות, כל אדם רשאי לעיין במידע שנאסף עליו, לבקש את תיקונו או מחיקתו, או להתנגד לשימוש במידע לצרכים שיווקיים. טופס זה מיועד להגיש בקשה רשמית למימוש זכויות אלה.
הנחיות למילוי הבקשה
- הבקשה נועדה למידע אישי שנאסף באתר talpanit-ai.com ובשירות טלפנית בלבד.
- ייתכן שתידרש הזדהות באמצעות כתובת הדוא"ל שלכם כדי לאמת את זהות מבקש/ת המידע.
- נענה לכל בקשה תוך 30 יום ממועד קבלתה, בהתאם לחוק.
- ייתכן שנשמור עותק של המידע ככל שנדרש לצרכים חוקיים (חשבוניות, תיעוד עסקאות וכו').
הצהרה
בשליחת טופס זה אני מצהיר/ה כי המידע שמסרתי נכון ומעודכן, וכי אני מבקש/ת לממש את זכויותיי בהתאם לחוק הגנת הפרטיות.

Instructions for Completing the Request
- This request applies only to personal information collected on the talpanit-ai.com website and the Talpanit service.
- You may be required to verify your identity via the email address associated with your account.
- We will respond to every request within 30 days of receipt, as required by law.
- We may retain a copy of the information as required for legal purposes (invoices, transaction records, etc.).
